This RN Case Manager position is per-diem (as needed), 10 hour shifts, working days at Northwest Medical Center.
Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Typically reports to a head of a department/unit.Qualifications
Associate's degree.Master's/Bachelor's Degree preferred.
Five (5) yearsexperience preferred. 2 years ofclinical/hospital experience or 2 years of experience in like areas required.
Able to interviewothers, strong analytical and organizational skills and team facilitation andmeeting skills.
Current Arizona RNlicense or compact state license and American Heart Association (AHA) BLScertification.
