Job Description

The LPN in theOutpatient Wound Care Clinic participates in the delivery of patient care undersupervision of an RN.  May be assigned toa non-nursing division but reports for clinical issues to a nurse. Assists physiciansduring examinations and procedures. Promotes patients' independence byestablishing patient care goals and teaching patients and families tounderstand conditions, medications, and self-care skills.


Provides competent,safe, and timely care based on the age of the patient within the scope ofpractice.

Accurately obtainsand legibly documents patient vital signs physical exams and comprehensivewound assessments.

Participates in theeducation, development and implementation of the patient care plan incollaboration with and under the direction of the RegisteredNurse.        

Monitors clinicalcondition of patients and reports changes to appropriate personnel in a timelymanner.                                                

Ensures accurate,timely communication among caregivers, to include appropriate chain of commandwhen indicated.         

Orients, instructs,and trains assigned personnel and/or students.

Participates inaudits, inventory counts and maintaining appropriate supply stock in theclinic.                

Assumesresponsibility for appropriate delegation of patient care tasks and overseescare provided by delegates.  Adheres tominimal patient load requirements.

Timely reporting,reviewing and documenting of all orders and unexpected incidents or adverseevents, including incident reports as applicable.        

Enhancesprofessional growth and development through participation in educationalprograms, current literature, in-services, andmeetings.        

Performs other jobduties relevant to the position asassigned.        




Qualifications

Education:                

Post High SchoolVocational/Specialized Training in Practical Nursingrequired.                                                

Experience:                

One (1) year ofclinical experience in area of expertisepreferred.                                                

Skills:                

As defined inCompetencyDocumentation.                                                

Licenses/Certifications:

Current LPN licensein Arizona 

BLS HealthcareProvider certification (Must be American Heart Associate)
