Job Description

Northwest Medical Center is a 300-bed hospital offering a full range of healthcare services including emergency services, accredited chest pain and heart failure programs, neurology and neurosurgery, outpatient imaging, a nationally accredited surgical weight loss program, total joint program, spine program and stroke center (recognized with Gold Seal designations by The Joint Commission), robotic surgery, GI lab, a free-standing women’s center, inpatient rehabilitation facility, wound care center, four urgent care facilities, and three ambulatory surgery centers. Coordinates andleads all activities for Service Line programs using an interdisciplinaryapproach. Responsibility for the operational and quality outcomes of serviceline for the defined population.

  • Assesses and monitors the continuity of care for the defined patient population including but not limited to coordination of daily rounds and multi-disciplinary rounds when indicated. Expedites and coordinates the delivery of services to facilitate a patient's progress through the continuum of care, from point of first contact, through the hospitalization period and post discharge to achieve desired  outcomes.
  • Assists in the development of patient care protocols and in updating patient care procedures for Service Line patients based on current literature/trends and evidence-based practice.
  • Assumes leadership role to facilitate interdisciplinary communication including coordinating regular Service Line meetings.
  • Functions as a clinical liaison and resource for the hospital program including serving as a liaison between physician/physicians offices.
  • Works with appropriate staff to educate the clinical staff regarding the special needs of  Service Line patients.
  • Analyze patient satisfaction data specific to service line patients and meets identified targets for performance.
  • Maintains appropriate credentialing and/or survey readiness for Service Line.
  • Teaches any Service Line classes and assesses impact of education on patient outcomes. Creates and updates patient education information and acts as community liaison.
  • In coordination with the Performance Improvement Department, develops and maintains data base for tracking outcomes and initiates performance improvement processes to correct variances.
  • Works with the medical staff and , if present, Physician Advisor for Service Line to establish collaborative relationship with medical staff for Service Line.



Associate Degreerequired. Bachelor's Degree preferred.


Three (3) yearsexperience in clinical setting specific to defined patient population stronglypreferred.  Experience with programdevelopment, third party payers and marketing preferred.                                                 


Compile statistics;computer literacy, data entry, retrieval, reporting; input data into computerprograms; interview others; research information; strong analytical andorganizational skills; and team facilitation and meeting skills.  Excellent human relations and oral/writtencommunications skills.  Computer skillsincluding word, excel, and graphic. Spanish speaking preferred. Physicianliaison.                                                


"Required:Nursing or other clinical licensure (i.e. PT,OT, ST, PTA) and American HeartAssociation (AHA) BLS.

Preferred: NAOM orclinical specialtycertification."
